I am recruiting for a Nurse Case Manager with a private company in Central London.
Duties of a Nurse Case Manager
The new Nurse Case Manager will be employed to ensure that outstanding quality standards of care are delivered by preparing risk assessments and providing outstanding bespoke care plans for clients ensuring that they are regularly updated to reflect any changes to the service. You will be working closely with the management team and in the field to ensure that qualified Carers and Nurses are assigned to Clients and outstanding levels of customer service are delivered.
Am I the right person for this role?
A minimum formal Nursing Qualification is essential. A valid NMC registration and PIN with no restrictions on practice. Experience of planning, implementing and evaluating care plans and treatments of clients.
Experience providing leadership to healthcare staff on compliance with:
o CQC regulations
o Health and Social Care Act 2008 (Regulated Activities) Regulations 2014
o Mental Capacity Act (MCA) 2005 or equivalent framework
Involvement managing a team of multidisciplinary healthcare professionals. Experience with managing multiple staff rotas and rota management software. Previous management of complaints and incidents or potential Safeguarding issues ensuring a consistent safe service in line with the CQC Fundamental Standards. Experience acting as a senior clinical team member of an on-call/emergency team.
Excellent IT skills including knowledge using Microsoft Office applications or similar software programmes as appropriate and excellent knowledge of our specific software for the bookings system
